Legacy Healthcare Services, Inc. is seeking a compassionate Physical Therapist to contribute to our senior care teams.

You will deliver rehabilitative services, develop and adjust treatment plans, and participate in clinical conferences to coordinate patient care. This role emphasizes individualized private sessions, functional-based therapy, and opportunities for growth within a not-for-profit provider serving senior living communities across the U.S.

Qualifications

• Degree in the area of discipline is required.
• Excellent interpersonal, fiscal, and clinical skills are essential.
• Computer proficiency is required for documentation and EHR use.
• Current Physical Therapy license in state of practice, or ability to obtain prior to starting employment.

Responsibilities

• Provide rehabilitative services to residents under medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care for patients.
• Treat residents one-on-one with individualized attention through private treatment sessions.
• Demonstrate functional-based treatment approaches and tailor sessions to individual needs.
