Legacy Healthcare Services, Inc. is hiring a Physical Therapist to deliver individualized rehabilitation services in a senior living setting. You will plan and implement treatment programs based on clinical findings, working closely with a coordinated care team to optimize patient outcomes.

Qualifications

• Degree in the area of discipline (Physical Therapy)
• Valid license or ability to obtain licensure before start
• Excellent interpersonal, fiscal, and clinical skills
• Computer proficiency and comfort with basic software

Responsibilities

• Provide rehabilitative services to residents under medical orders as part of a coordinated care team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate plan of care.
• Treat residents one on one in private treatment sessions with a functional-based approach.
• Individualize treatment sessions to meet each patient’s needs
