Job summary

Legacy Healthcare Services, Inc. is seeking a Physical Therapist to join a dedicated team serving senior living residents. You’ll deliver individualized, functional-based therapy and build strong relationships with patients to improve quality of life.

Plan and adjust treatment programs based on clinical findings, participate in clinical conferences, and provide one-on-one sessions with a focus on compassionate, patient-centered care in a Monday–Friday schedule.

Qualifications

• Degree in physical therapy or related discipline.
• Current Physical Therapy license in state of practice, or ability to obtain before employment.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer literacy and proficiency.

Responsibilities

• Provide rehabilitative services to residents under medical orders as part of a team.
• Plan initial and ongoing treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the care plan.
• Treat residents one-on-one with private sessions and individualized treatment.
• Tailor sessions to each patient’s functional goals.
