Job summary

Legacy Healthcare Services, Inc. seeks a Physical Therapist to deliver individualized rehabilitation care to senior residents and to collaborate with a multidisciplinary team.

The role centers on one-on-one treatment sessions, planning treatment programs, and contributing to care plans in a supportive, not-for-profit environment. A Monday–Friday schedule and comprehensive benefits are offered.

Qualifications

• Degree in physical therapy or related discipline.
• Current Physical Therapy license in state of practice, or eligibility to obtain prior to starting employment.
• Excellent interpersonal, fiscal, and clinical skills; ability to work with computers.

Responsibilities

• Provide rehabilitative services to residents under medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care for patients.
• Treat residents one on one with individualized attention through private sessions using a functional-based approach.
