Qualifications

• Active New York State License (LMSW/MHC-LP).
• Master's in Social Work or related graduate degree.
• LMSW, LSW, LAC, MHC or equivalent.
• Proficient with Google Suite, company-wide platforms and EHR platforms when applicable.
• Intermediate computer skills and technical aptitude.

Responsibilities

• Provide high-quality clinical care in telehealth and hybrid settings.
• Maintain a consistent weekly patient care schedule in line with productivity expectations.
• Follow care model practices emphasizing continuity of care and frequent follow-ups.
• Provide clinical support and counseling to diverse patient populations via telehealth.
• Coach patients to manage ongoing health conditions and adopt positive behavior changes.
• Promote a friendly, collaborative work culture that supports growth and well-being.
• Participate in weekly supervision and group supervision for case discussions and knowledge sharing.
• Collaborate within a multidisciplinary clinical community to ensure coordinated care.
