Qualifications

• Active New York State license (LMSW/LSW/MHC-LP) or equivalent.
• Master's degree in Social Work, Mental Health Counseling, Counseling Psychology, or related field.
• Proficient with diverse technology platforms and EHR systems.
• Intermediate computer skills and strong telehealth capabilities.

Responsibilities

• Provide clinical care via telehealth and hybrid formats.
• Maintain a consistent weekly patient care schedule aligned with productivity expectations.
• Follow care model emphasizing continuity of care and high-frequency follow-ups.
• Offer counseling to diverse patient populations in a telehealth environment.
