Job summary

AccessHealth is seeking Licensed Practical Nurses (LPNs) to join the Transition Team, delivering immediate home-based care during transitional periods, while permanent staff are recruited.

You will assess patients, update care plans, administer treatments, and respond to condition changes, coordinating with physicians and caregivers to ensure smooth transitions back home.

Qualifications

• Current LPN license in the practice state.
• Valid health certificate, PPD or chest X-ray, and BLS certification.
• Ability to travel approximately 45 minutes to an hour.
• Experience with pediatrics and adults; training available for varied acuity patients (e.g., g-tube, trach, vent).
• Preferred: One year of LPN experience.

Responsibilities

• Assess, plan, and evaluate patient care using the nursing process.
