Maxim Healthcare is hiring Licensed Practical Nurses (LPNs) to join our Transition Team. The team supports patients transitioning home during the provisional period, providing home-based nursing care for 1–90 days as permanent staff are recruited.

Role requires current LPN license, Health Certificate, PPD/Chest X-ray, and BLS; travel is required. Competitive hourly pay, flexible scheduling, and company benefits are offered to qualified candidates.

Qualifications

• Current Licensed Practical Nurse (LPN) license is required in the state of practice.
• Current Health Certificate (per contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS certification.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Initiate and update plans of care; request revisions as patient needs change.
• Provide therapeutic, preventive, and rehabilitative nursing interventions.
