Job summary

Maxim Healthcare is seeking Licensed Practical Nurses (LPNs) to join the Transition Team in a role that supports patients transitioning home while permanent nursing staff are recruited. The team operates in patients' homes for an initial period, coordinating care and training new caregivers.

Responsibilities include using the nursing process, planning and adjusting care, administering medications, and educating patients and families.

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of practice
• Current Health Certificate (per contract or state regulation)
• Current PPD or Chest X-Ray
• Current BLS certification
• Ability to travel 45 minutes to an hour
• Ability to work with pediatrics and adults
• Ability to care for (training available) patients with varied acuity (I.e. g-tube, trach, vent)
• One year of LPN experience preferred

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care
• Initiate and update plans of care as directed; request provider revisions as patient needs change
• Provide therapeutic, preventive, and rehabilitative nursing interventions based on diagnosis and goals
• Respond to changes in patient condition and report to physicians or supervisors as appropriate
• Administer prescribed medications and treatments and document patient response
• Identify opportunities for patient/family education, care coordination, goal setting, and discharge planning
