Job summary

Maxim Healthcare Services, Inc. is seeking Licensed Practical Nurses (LPNs) to join the Transition Team, a home-care unit that supports patients transitioning home after referral.

You will provide care for all authorized hours in the home during a provisional period, and collaborate with office staff recruiting permanent caregivers. You’ll work with pediatrics and adults, travel to patient homes, and train alongside Transition Team nurses before assuming independent care.

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of practice.
• Current Health Certificate (per contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS certification.
• Ability to travel 45 minutes to an hour.
• Ability to work with pediatrics and adults.
• Ability to care for patients with varied acuity (training available).
• One year of LPN experience preferred.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Initiate and update plans of care as directed; request provider revisions as patient needs change.
• Provide therapeutic, preventive, and rehabilitative nursing interventions based on diagnosis and goals.
• Respond to changes in patient condition and report to physicians or supervisors as appropriate.
• Administer prescribed medications and treatments and document patient response.
• Identify opportunities for patient/family education, care coordination, goal setting, and discharge planning.
