Job summary

Maxim Healthcare Services, Inc. is seeking Licensed Practical Nurses (LPNs) for its Transition Team to support patients transitioning home. The role covers a provisional period of 1–90 days with in-home nursing care during authorized hours.

Qualifications

• Current LPN license in the state of practice.
• Current Health Certificate (per contract or state regulation).
• Current PPD or Chest X-Ray and BLS certification.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Initiate and update plans of care as directed; request provider revisions as patient needs change.
• Provide therapeutic, preventive, and rehabilitative nursing interventions based on diagnosis and goals.
• Respond to changes in patient condition and report to physicians or supervisors as appropriate.
• Administer prescribed medications and treatments and document patient response.
• Identify opportunities for patient/family education, care coordination, goal setting, and discharge planning.
