Job summary

Epiphany Wellness LLC in Tennessee is seeking a Primary Therapist to work with clients to determine rehabilitation needs and provide individualized treatment through structured and holistic healing. You will manage a patient caseload and provide group, individual, and family therapy, conduct psychosocial assessments, and communicate with families and referral sources as needed.

This full-time position offers Tue-Sat schedule 8:30 to 4:30 pm CST, with comprehensive benefits and growth

Qualifications

• Master’s degree in social work, psychology or related human services field required.
• A minimum of 1 year of mental health counseling experience is required.
• Candidates pending TN Clinical licensure are encouraged to apply.
• Clinical Licensure preferred, not required: Mental Health, SUD.
• CPR Certification preferred.

Responsibilities

• Lead group counseling sessions and facilitate peer support.
• Provide individual therapy to clients to address recovery goals.
• Perform crisis intervention as needed to ensure safety.
• Maintain accurate documentation of client progress.
• Develop and update individualized treatment plans.
• Coordinate case management with families and referral sources.

We are looking for a Primary Therapist who will work closely with clients to determine their rehabilitation needs and provide individualized treatment through structured and holistic healing.

This role supports clients' foundational well-being and helps them build the skills needed to overcome challenges, heal, and grow to sustain long-term sobriety.

The Therapist is responsible for performing a wide variety of patient care activities. They will manage a patient caseload and provide group, individual, and family therapy.

The Therapist will conduct psychosocial and biopsychosocial assessments and maintains primary responsibility for communication with the family, referral source, and other service providers as needed.
