Unity Hospice is seeking a Primary Registered Nurse (RN) to serve as the lead case manager for patients in home and facility settings in the Sturgeon Bay territory. The role involves coordinating with float nurses and an interdisciplinary team to deliver consistent, patient-centered care, with autonomy and teamwork at its core.
The position is full-time (36 hours/week) across four 9-hour shifts, with weekend rotation and on-call duties. Occasional travel and holiday coverage may be required.

Schedule: Full-time, 36 hours per week, consisting of four 9-hour shifts (typically 7:30 a.m.–4:30 p.m.). Weekend rotation is every 3rd weekend (currently every 8th weekend), with 12-hour shifts from 11:00 a.m.–11:00 p.m. on Saturday and Sunday. Includes a shared 10–15-day on-call rotation and 2–3 holidays per year. Holiday coverage may require working hours outside of your regular scheduled shift.
As a Primary Registered Nurse, you will serve as the lead case manager for patients in both home and facility settings. You’ll partner closely with float nurses and an interdisciplinary team to deliver consistent, high-quality, patient-centered care.

Territory: Sturgeon Bay area (with occasional travel outside the territory)
***During periods when your caseload is lighter, you will be expected to support your team by assisting with patient visits and transitions, serving as a Float Registered Nurse as needed.
