Description
TRIAGE REGISTERED NURSE (RN)

The Triage Registered Nurse is responsible for responding to and triaging telephone calls and other contacts related to patient needs. This includes conducting patient assessments and providing safe and effective telephone advice, care instructions, and direction to the appropriate level of care. The Triage Registered Nurse works collaboratively with members of the interdisciplinary team to provide exceptional customer service and achieve high-quality patient care standards.

Schedule: 3:00 p.m.–11:00 p.m., four days per week, primarily working as the Triage RN, with the flexibility to work some shifts in the community as needed. Weekend coverage is required every fourth weekend from 8:00 a.m.–8:00 p.m. in Triage, which may be worked from home. The position also includes an on-call rotation of approximately 10–15 shifts per year and coverage for approximately 2–3 holidays per year.

Key Job Duties
  1. Telephone triage and follow-up to patient care-related contacts
  2. Responsible for providing skilled nursing assessment and intervention
  3. Provide updates to the interdisciplinary team and attending physicians
  4. Update Plan of Care as necessary
  5. Provide education to patients and family members on symptom management, disease progression, medication, and other associated patient care needs
  6. Record and maintain complete and accurate records of patient and physician communications
  7. Fax/call new and refill prescriptions according to physician orders
  8. Record all received orders to the Electronic Medical Record and initiate follow-up as needed
  9. Respond to patient needs according to medical priority.
  10. Provide assistance and direction to patient care staff as needed
  11. Coordinate volunteer deliveries
  12. Abide by Standards of Nursing Practice as outlined in Wisconsin Administrative Code
  13. Comply with all applicable local, state and federal laws and regulations as well as policies and procedures of Uni
  14. Satisfy mandatory training requirements regarding local, state and federal regulations and/or Unity’s policies and procedures
  15. Consistently and concisely document activity according to department protocols and standard
  16. Promote and maintain confidentiality
  17. Perform other job-related duties as assigned
  18. Maintain regular and reliable attendance
  19. Shares in providing seven days-a-week coverage
Qualifications
  1. Current unrestricted Registered Nurse licensure from the State of Wisconsin
  2. Minimum of two years’ experience as a professional nurse; one to three years of hospice experience preferred
  3. Excellent critical thinking, interpersonal relationship building, and technical skills
  4. Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage

Requirements
  1. Associate degree in Nursing required, bachelor’s degree preferred.
