Job summary

Oak Street Health, part of CVS Health, seeks a Registered Nurse (RN) to join a team delivering high-touch primary care in a full-time role. You will coordinate care plans, outreach, and coordination across care teams to ensure quality patient outcomes.

The ideal candidate has 2+ years of outpatient nursing experience, an active RN license, CPR, EMR skills, and US work authorization. Travel between facilities in the Chicagoland area is required, with comprehensive CVS Health benefits.

Qualifications

• 2+ years of relevant experience in a primary care or outpatient setting
• Current state license to practice as a Registered Nurse
• Exceptional communication skills and customer service orientation
• CPR Certification and EMR experience
• US work authorization and ability to travel between facilities

Responsibilities

• In-center care support and triage for patients, scheduling visits and outreach
• Provide patient education on chronic conditions, in person or group settings
• Monitor and coordinate delivery of the care plan
• Collaborate with care teams to support transitions from hospital to home
• Coordinate referrals and resources to support cost-effective, high-quality outcomes
• Develop relationships with patients and caregivers to support preventive care
• Conduct medication and testing authorizations
• Engage with hospitals and SNFs to plan care
• Identify opportunities for workflow improvements
• Report admissions and progress to OSH leadership
• Other duties as assigned
