Qualifications

• Valid RN license in the state of New York.
• 3+ years of clinical experience as an RN.
• 2+ years of case‑management, discharge planning, or home‑care coordination experience.
• 1+ year of experience working with adult populations.

Responsibilities

• Conducts a proactive plan of care to address identified issues, improving short- and long-term outcomes.
• Utilizes clinical tools and data to assess member needs and benefits.
• Applies medical judgment to design interventions that reduce risk and barrier factors, addressing complex health and social issues.
• Performs assessments that incorporate data from multiple sources, including claims, to manage comorbidities and multiple diagnoses.
• Uses a holistic approach to determine referrals to medical resources and interdisciplinary team members.
• Collaborates with managers and key stakeholders to overcome limitations, achieve goals, and facilitate case conferences.
• Implements case‑management processes in compliance with regulatory and organizational policies.
• Uses motivational interviewing skills to ensure member engagement and assess health status and preferences.
