Job summary

Urban Health Plan in the Bronx seeks Physician Assistants and/or Nurse Practitioners to join our primary care team. You will provide comprehensive primary care services, conduct exams, order tests, manage labs, and collaborate with clinicians to deliver patient-centered care in a community health setting.

You will work with a multidisciplinary team, follow procedures, educate patients, and contribute to quality improvement and preventive care initiatives.

Qualifications

• Completion of an approved/accredited educational program for Physician Assistants
• NY State license, CDS, DEA, and BLS
• Ability to effectively communicate, follow established procedures and protocols
• Understanding of and passion for Community Healthcare is preferred

Responsibilities

Job Description

We are looking for Physician Assistants and/or Nurse Practitioners to provide primary care services as part of our primary care team. The Physician Assistant/Nurse Practitioner will work with a multi-disciplinary team to care for and provide medical services to patients, including examining/conducting tests; analyze records, reports, and test results; to prevent and/or diagnose illness, disease or injury; prescribe/administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, and/or injury; explain procedures and test results on prescribed treatment plan; manage labs, referrals. The Physician Assistant will collaborate with other physician assistants, nurse practitioners, physicians, and other support staff as part of the clinical team.Perform other job-related duties as required by departmental needs/demands.

Experience and Skills

• Completion of an approved/accredited educational program for Physician Assistants

• Valid NY State license, CDS, DEA, andBLS

• Ability to effectively communicate, follow established procedures and protocols

• Understanding of and passion for Community Healthcare is preferred
