Job summary

Urban Health Plan seeks a dedicated Geriatrician to deliver integrated medical care in our community health centers. You will diagnose, treat, and prevent illnesses in elderly patients, both in home visits and in the office, and collaborate with NP/PA teams to coordinate care.

Required are an MD/DO from an accredited school, a geriatrics fellowship, board certification, and NY license with CDS/DEA/BLS. We offer comprehensive benefits and a patient-centered, growth-focused environment.

Qualifications

• MD/DO degree from an accredited medical school and completion of accredited residency program.
• Fellowship in Geriatrics.
• Board Certified.
• Valid NY State license CDS, DEA, and BLS.

Responsibilities

• Provide medical care to treat, prevent and diagnose illness, disease or injury in geriatric patients, and explain procedures and test results.
• Collaborate with non-physician providers (NP, PA, CNM, etc.) and perform home visits and in-office visits as required.
• Educate patients about diet, activity, hygiene, and disease prevention.

Job Description:

We employ community-driven and highly skilled clinicians to provide integrated care in our Health Centers. We are seeking new associates to join our team in the capacity of Geriatrician. The Geriatrician will provide medical care to treat, prevent and/or diagnose illness, disease, or injury. The physician will examine or conduct tests to provide information on medical status, analyze records, reports, test results, and/or examination information to diagnose medical condition of patient, prescribe/administer treatment, therapy, medication and/or vaccination. The physician will also focus on explaining procedures and discussing test results on prescribed treatments with patient and advise patients and community concerning diet, activity, hygiene, and disease prevention. The physician will collaborate with and non-physician providers (NP, PA, CNM, etc). The physician will perform other job-related duties as required by departmental needs/demands.Home visits and in office visits.

Experience and Skills:

• MD/DO degree from an accredited medical school and completion of accredited residency program
• Completion of Fellowship in Geriatrics
• Board Certified
• Valid NY State license, CDS, DEA, and BLS*Understanding of and passion for Community Healthcare is preferred
