Job summary

Urban Health Plan in the Bronx is seeking Physician Assistants and/or Nurse Practitioners to join our virtual primary care team providing family medicine services. You will work with a multidisciplinary group to care for patients, conduct tests, review results, and manage referrals as part of a clinical team.

Job Description:

We are looking for Physician Assistants and/or Nurse Practitioners to provide family medicine primary care services as part of our virtual primary care team. The Physician Assistant/Nurse Practitioner will work with a multi-disciplinary team to care for and provide medical services to patients, including examining/conducting tests; analyze records, reports, and test results; to prevent and/or diagnose illness, disease or injury; prescribe/administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, and/or injury; explain procedures and test results on prescribed treatment plan; manage labs, referrals. The Physician Assistant/Nurse Practitioner will collaborate with other physician assistants, nurse practitioners, physicians, and other support staff as part of the clinical team.Perform other job-related duties as required by departmental needs/demands.

Experience and Skills:

• Completion of an approved/accredited educational program for Physician Assistants

• Valid NY State license, CDS, DEA, andBLS

• Ability to effectively communicate, follow established procedures and protocols

• Understanding of and passion for Community Healthcare is preferred
