Job summary

Landwell Health seeks a licensed NP or PA to deliver primary and preventive care to diverse patients across ages in a community setting. This is a Monday–Friday, 9 AM–5 PM role with no weekend shifts, anchored in Brooklyn with occasional Queens days.

You'll work closely with physicians and a multidisciplinary team to provide comprehensive care, educate patients, manage acute and chronic conditions, and coordinate referrals to ensure continuous, accessible care for families.

What You'll Do

As part of the clinical team, you'll provide comprehensive primary care while working closely with physicians, clinical staff, and other healthcare professionals.

Responsibilities include:

• Evaluate, diagnose, and treat patients across multiple age groups
• Perform routine physicals and preventive health visits
• Develop individualized treatment plans
• Diagnose and manage acute and chronic health conditions
• Prescribe medications within your scope of practice
• Order and interpret appropriate diagnostic testing
• Provide preventive care and health education
• Educate patients on chronic disease management, nutrition, medications, and healthy lifestyle choices
• Coordinate referrals and follow-up care when specialty services are needed
• Collaborate with physicians and other members of the clinical team
• Maintain accurate and timely clinical documentation
• Support continuity of care and help patients navigate healthcare services
• Participate in clinical quality and patient-care initiatives

What We're Looking For

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in New York
• Strong primary care or related clinical experience
• Comfortable providing care to patients across multiple age groups
• Strong assessment, diagnosis, and treatment skills
• Comfortable managing both acute and chronic conditions
• Strong patient education and communication skills
• Interest in community-based healthcare
• Ability to work collaboratively within a multidisciplinary clinical team

Experience in primary care, family medicine, internal medicine, community health, ambulatory care, or a similar setting is particularly relevant.
