Job summary

Urban Health Plan is seeking Physician Assistants and/or Nurse Practitioners to join our virtual family medicine primary care team. You will work with a multidisciplinary group to deliver medical services, including exams, tests, treatment, and referrals, in a patient-centered environment.

Requirements include a NY State license, CDS, DEA, and BLS, with a passion for community healthcare. We offer a supportive, innovative setting, with strong benefits and opportunities for growth.

Qualifications

• Completion of an approved/accredited educational program for Physician Assistants
• Valid NY State license, CDS, DEA, and BLS
• Ability to effectively communicate, follow established procedures and protocols
• Understanding of and passion for Community Healthcare is preferred

Responsibilities

• Provide family medicine primary care services as part of our virtual primary care team
• Examine, conduct tests, and analyze records to prevent or diagnose illness
• Prescribe/ administer treatment, therapy, medication, vaccination, and other care; manage labs and referrals
• Collaborate with other clinicians and support staff as part of the clinical team

Job Description:We are looking forPhysician Assistants and/or Nurse Practitioners to provide family medicine primary care services as part of our virtual primary care team. The Physician Assistant/Nurse Practitioner will work with a multi-disciplinary team to care for and provide medical services to patients, including examining/conducting tests; analyze records, reports, and test results; to prevent and/or diagnose illness, disease or injury; prescribe/administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, and/or injury; explain procedures and test results on prescribed treatment plan; manage labs, referrals. The Physician Assistant/Nurse Practitioner will collaborate with other physician assistants, nurse practitioners, physicians, and other support staff as part of the clinical team.Perform other job-related duties as required by departmental needs/demands.Experience and Skills:Completion of an approved/accredited educational program for Physician Assistants

Valid NY State license, CDS, DEA, andBLS

Ability to effectively communicate, follow established procedures and protocols

Understanding of and passion for Community Healthcare is preferred
