Job summary

Hometown Urgent Care & Occupational Health is seeking a compassionate Primary Care Provider/Advanced Practice Provider (NP or PA) licensed in Ohio to deliver comprehensive primary care across the lifespan. The role involves prevention, diagnosis, treatment, and patient education within a collaborative team-based environment.

You will conduct exams, manage conditions, and coordinate care with specialists while documenting with E-ClinicalWorks.

Qualifications

• Licensed NP-C/FNP or PA-C in Ohio.
• Experience in primary care, family medicine, or internal medicine.

Responsibilities

• Conduct comprehensive physical exams, patient histories, and assessments.
• Diagnose and manage a wide range of conditions across ages.
• Provide primary care across multiple specialties and handle in-scope procedures.
• Collaborate with ENT, Surgical, and Medical Subspecialties for continuity of care.
• Utilize E-ClinicalWorks for compliant, patient-centered documentation.
• Provide emergency and urgent interventions when needed.
• Educate patients and families on treatment plans, prevention, and lifestyle changes.

What You'll Do

• Perform in-scope procedures
• Collaborate closely with specialists in ENT, Surgical, and Medical Subspecialties to ensure continuity and coordination of care
• Utilize E-ClinicalWorks for efficient, compliant, and patient-centered documentation
• Provide emergency and urgent interventions when necessary
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

Requirements

• Certified Nurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C) licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills—your ability to connect with patients matters as much as your clinical expertise
• Commitment to teamwork, continuous learning, and high-quality care
• E-Clinical Works EMR experience a PLUS!
