The Department of Veterans Affairs seeks a Primary Care Physician to lead a PACT team or serve as a float/gap provider delivering comprehensive, collaborative care to Veterans. The role emphasizes care coordination, patient engagement, and timely access within regular clinic hours.

Responsibilities

• Ongoing, continuous care of one or more assigned panel(s) of Veterans.
• Utilizing registries and tools to identify and intervene with patients and cohorts.
• Ensure evaluation and access for patients assigned to the panel.
• Provide health coaching on wellness, disease prevention, and self-management.
• Engage patients and support persons in VA care and decision making.
• Communicate effectively with team members and patients to convey clinical information.
• Provide care commensurate with licensure and clinical privileges.
