Job summary

The VA Western New York Healthcare System in Buffalo is seeking a Full-Time Physician for its Primary Care department to deliver outpatient care within the Patient Aligned Care Team (PACT) model. Qualifications

• MD/DO degree from an approved medical school
• Current full and unrestricted medical license in the United States
• Board eligibility or board certification in Internal Medicine or Family Medicine
• Proficiency in spoken and written English

Responsibilities

• Provide outpatient care for primary care patients within the PACT model
• Manage a patient panel and coordinate with the care team
• Prescribe medications and order diagnostic studies as appropriate
• Complete documentation within 48 hours of visits
• Address clinical reminders and participate in team communications
