Qualifications

• Full-time Family Practice or Internal Medicine trained physician.
• Recent experience in VA outpatient PACT panel management.
• Experience working in a Complex PACT (COM-PACT) Primary Care setting.

Responsibilities

• Provide comprehensive gender-specific care under the PACT model.
• Manage a panel of patients with complex chronic conditions per PCMM guidelines.
• Coordinate care with an interdisciplinary team to reduce ED visits and readmissions.
• Ensure timely documentation and adherence to VA policies.
