We are seeking a dedicated and patient-focused Primary Care Physician (MD/DO) to join our innovative primary care network, providing value-based healthcare services to Medicare populations in York, PA. Primary Care Physician (MD/DO) – Summary

• Deliver value-based primary care to Medicare patient populations in a collaborative, patient-centered environment.

Duties & Responsibilities

• Provide comprehensive primary care services to a panel of adult and geriatric patients.
• Conduct regular and diagnostic visits, with appointment durations of 20 minutes for standard visits and 40 minutes for new patient or full diagnostic appointments.
• Collaborate with a multidisciplinary care team, including scribes, medical assistants, and social workers, to coordinate patient care and enhance clinical outcomes.
• Utilize advanced electronic medical record (EMR) systems, such as Canopy and Greenway, to document patient encounters and manage care.
• Participate in value-based care initiatives focused on preventive medicine, chronic disease management, and reducing unnecessary hospital admissions.
• Engage patients in health education, wellness activities, and community programs such as exercise, yoga, dance, and wellness events.
• Ensure compliance with all regulatory and ethical standards in the practice of medicine.

Qualifications & Requirements

• Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
• Board certification or board eligibility in Internal Medicine or Family Medicine.
• Active, unrestricted state medical license (no probationary status).
• Fellowship training in Geriatrics is preferred but not required.
• Residency completed; residents are not eligible for this role.
