• Work alongside nurse practitioners/physician assistants, medical assistants, social workers, and in-house lab and pharmacy support.
• Participate in an integrated, team-based care model to coordinate referrals and ensure comprehensive care.
• Utilize electronic medical records (EMR) efficiently and adhere to documentation best practices.
• Engage in value-based care strategies, including preventive screenings, chronic disease follow-up, and population health initiatives.
• Attend and complete a 3-week onboarding program covering value-based care principles, EMR optimization, and workflow integration.
• Contribute to a culture of continuous improvement, quality, and patient satisfaction.
