Job summary

Allegheny Health Network is seeking a Physician Assistant/NP to provide primary care services under physician supervision, with a focus on population health and value-based outcomes. The role includes coordinating care, prescribing medications, and contributing to quality initiatives.

In addition to base compensation, the position offers an annual quality-based bonus. Weekend and on-call rotation may be required as part of the care delivery team.

Qualifications

• Duties include patient assessment, therapy planning, orders, and medical record documentation.
• Coordinate care with physicians, family, and care teams.
• Provide education to patients and families.
• Participate in performance and quality improvement initiatives.
• Compliance with HIPAA and company policies.
• On-call coverage and weekend rotation as required.

Responsibilities

• Record medical history, perform exams, develop care plans, and document records.
• Coordinate care with the interdisciplinary team.
• Monitor and coordinate periodic physician evaluations.
• Prepare prescription orders per protocol.
• Educate patients on health, medications, and resources.
• Assist with triage, follow-up, and quality initiatives.
• Support outpatient/specialty duties as needed.
