Qualifications

• MSN from an approved program and NP licensure in the practicing state.
• National Board Certification for Nurse Practitioners.
• Prescriptive Authority within state requirements; CPR certification.

Responsibilities

• Provide primary healthcare for non-acute or chronic conditions in assigned populations.
• Record patient history and physical assessments; develop therapeutic care plans.
• Order and interpret diagnostic tests; document medical records accurately.
• Coordinate patient care with physicians, families, and inter-disciplinary teams.
• Educate patients and families on health, medications, and preventive care.
• Triage and manage outpatient concerns; participate in quality improvement initiatives.
