Job summary

• Perform history taking, physical examination, order labs and diagnostic studies, and formulate treatment plans.
• Coordinate care with multidisciplinary team including social work, pharmacy, mental health, nutrition and others.
• Document patient care in clinical records and refer to specialists when necessary.
• Lead and support patient advocacy and address patient concerns and inquiries.
