The U.S. Department of Veterans Affairs Durham PACT Outpatient Clinic seeks a qualified registered nurse to provide primary and specialty care coordination for Veterans.

You will work with attending physicians, mid-levels, nurses, pharmacists, social workers, and support staff to deliver comprehensive care across the continuum. Responsibilities include patient education, self-management coaching, and involvement in quality improvement initiatives.

Formación

• English language proficiency required for direct patient care.
• Graduate of a nursing program approved by a recognized accrediting agency (ACEN or CCNE).
• Current, full, active, and unrestricted RN license in the United States or District of Columbia.

Responsabilidades

• Coordinates care focusing on patient education, self-management, and satisfaction throughout the continuum of care.
• Collaborates with interdisciplinary team to influence care outcomes.
• Participates in unit/program quality improvement and customer service programs.
• Provides direct Veteran care and care coordination within the Primary Care Department.
• Promotes health education and coaching on wellness and chronic disease management.
