Job summary

Greenville Outpatient Clinic is seeking a Registered Nurse to join the Patient Aligned Care Team (PACT) and support comprehensive primary care for Veterans across a broad range of conditions.

The RN will lead nursing staff, conduct patient intake and triage, perform health assessments, plan and coordinate care, provide health coaching, and promote evidence-based, patient‑centered care within an interdisciplinary team.

Qualifications

• 2-3 years Primary Care experience preferred.
• Direct patient care within the last 2 years preferred.

Responsibilities

• Oversee nursing staff and patient intake.
• Provide health coaching and disease prevention.
• Coordinate care and promote safe outpatient services.
• Triage and take patient histories during visits and calls.
• Promote evidence-based-practice within the interdisciplinary team.
