Job summary

Qualifications

• English language proficiency per VA guidelines.
• Graduate of an accredited school of nursing (ACEN/CCNE) or equivalent.
• For foreign-trained nurses: licensure validation and state registration upon graduation.
• May start at GNT level if licensure is pending per VA grandfathering provisions.
• Reference VA Qualification Standards for nurse roles.

Responsibilities

• Coordinate comprehensive, patient-centered nursing care for panels of patients.
• Lead health promotion and chronic care management using evidence-based guidelines.
• Collaborate with PACT and external services to ensure timely, coordinated access to care.
• Educate patients and families and advocate for patient needs to improve outcomes.
• Promote continuity and quality of care across the care setting.

Education

Associate Degree in Nursing (ADN) or Diploma in Nursing

Bachelor of Science in Nursing (BSN)

Registered Nurse (state license)

Nurse with bridge program or coursework equivalent to bachelor’s in Nursing (optional)

Current, full, active, and unrestricted nursing registration in a U.S. state or DC
