U.S. Department of Veterans Affairs seeks an Outpatient Staff Registered Nurse for the Patient Aligned Care Team (PACT) in Farmington, NM.

The role focuses on leadership in primary care, care coordination for Veterans, and promoting wellness and disease management within the VA Health Care System. The position offers a Monday–Friday schedule, participation in a comprehensive benefits package, and opportunities to influence care delivery through evidence-based practices.

Qualifications

• Valid, full, and unrestricted RN license from any US state or territory.

Responsabilités

• Executes position responsibilities with leadership and creative approaches to primary care management.
• Provides gender-specific direct Veteran care and care coordination.
• Manages assigned panels, uses registries, and ensures evaluation and access to patient panels.
• Promotes health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
• Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and guidelines.
• Evaluates outpatient outcomes of evidence-based decisions and practice changes for primary care individuals, groups, and populations.
