Job summary

Oak Street Health in Providence, RI is seeking a Nurse Practitioner to provide effective value-based primary care to adults on Medicare. You will work independently with physicians on a collaborative care team and focus on building meaningful patient relationships.

Responsibilities include office visits, annual wellness visits, health risk assessments, care coordination, population health leadership, patient education, and involvement in promotional activities.

Qualifications

• Master of Science in Nursing or Doctor of Nursing Practice required.
• National certification in NP specialties listed: FNP, AGPCNP, ANP, GNP.
• Active, non-probationary NP license in the U.S.
• Active DEA license.
• US work authorization.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and HRAs.
• Care coordination with other providers, facilities, and agencies.
• Lead population health in coordination with the care team.
• Assist with phone triage and outreach.
