Job summary

Oak Street Health is seeking a Nurse Practitioner to provide effective, equitable value-based primary care to adults on Medicare. You will see patients independently, collaborate with physicians, and participate in a collaborative outpatient care team focused on population health.

Responsibilities include conducting visits, annual wellness exams, care coordination, patient education, and potential home visits.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult Nurse Practitioner, or Gerontological NP
• Active, non-probationary state NP license
• Active DEA license
• US Work Authorization

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with accurate documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
