Qualifications

• Master of Science in Nursing or higher required.
• National certification in an NP specialty (FNP/AGPCNP/ANP/Gerontological NP) required.
• Active, non-probationary state NP license and DEA license are required.

Responsibilities

• Provide effective and equitable value-based primary care to adults on Medicare.
• Collaborate with physicians and care team to coordinate care.
