Job summary

Qualifications

• Provides evidence-based medical care while meeting the needs of the patient and respecting diverse backgrounds.
• Collaborates with care team to develop a biopsychosocial plan of care including disease management, wellness, and prevention coaching.

Responsibilities

• Manages 12-16 patients per day providing primary care including preventive, chronic, and acute care.
• Participates in after-hours on-call rotation with other providers in the region.
• Documents and codes clinical care timely to reflect gap closures and risk adjustments.
• Leads team communications, process improvements, and quality assurance activities.
