Job summary

Vera Whole Health in Nokomis, FL is seeking a Nurse Practitioner, General / Primary Care, to provide comprehensive primary care across the lifespan in ambulatory and community settings.

You will evaluate, diagnose, and manage conditions, develop treatment plans, and collaborate with physicians and care teams to deliver evidence-based, preventive, and coordinated care for patients in a patient-centered environment.

Qualifications

• Master's-level clinical training within the APP scope.
• Ability to evaluate, diagnose, and manage acute and chronic conditions.
• Collaborate with physicians and interdisciplinary teams to coordinate care.

Responsibilities

• Evaluate, diagnose, and manage acute illnesses, chronic diseases, and preventive healthcare needs.
• Develop individualized treatment plans including medications, preventive services, lifestyle counseling, health promotion, immunizations, and follow-up.
• Diagnose and manage common primary care conditions including hypertension, diabetes, hyperlipidemia, asthma, COPD, obesity, thyroid disorders, depression, anxiety, musculoskeletal conditions, infectious diseases, and other illnesses.
• Order, perform, and interpret laboratory studies, diagnostic imaging, point-of-care testing, EKGs, and other diagnostics.
• Perform office-based procedures such as minor wound care, skin lesion management, injections, incision and drainage, and other approved procedures.
• Conduct annual wellness visits, preventive screenings, health risk assessments, chronic care management, and transitional care management.
• Coordinate referrals to specialty care, behavioral health, community resources, and ancillary services as indicated.
• Educate patients and caregivers on diagnoses, medications, preventive health, nutrition, exercise, and self-management.
• Document encounters and clinical decisions in the EHR in line with standards and regulations.
• Participate in quality improvement and population health initiatives to support excellence in primary care.
