Job summary

Vera Whole Health is seeking a Nurse Practitioner, General / Primary Care, to provide advanced practice primary care services across the lifespan in ambulatory settings. The role involves independent decision-making within APP scope and collaboration with physicians, nurses, and care teams to deliver high-quality, patient-centered care.

Qualifications

• Bilingual Spanish/English Master's or Doctor of Nursing Practice from an accredited NP program.
• Current, unrestricted NP license and Advanced Practice Registered Nurse licensure in the applicable state.
• National board certification as FNP/AGPCNP/AGACNP or other applicable certification.
• DEA registration and BLS certification; ACLS preferred based on assignment.
• Typically requires two or more years of advanced practice clinical experience in primary care or related ambulatory setting.
• Experience using electronic health records and population health platforms to support quality care.

Responsibilities

• Evaluate, diagnose, and manage patients across the lifespan in primary care.
• Develop individualized treatment plans including medications, preventive care, and counseling.
• Diagnose and manage common primary care conditions (hypertension, diabetes, lipids, etc.).
• Order, perform, and interpret labs, imaging, EKGs, and other diagnostics.
• Perform office-based procedures per scope and regulations.
• Conduct annual wellness visits and preventive services.
• Coordinate referrals to specialty care and community resources.
• Educate patients and caregivers on diagnoses, meds, and self-management.
• Document encounters and outcomes in the EHR with compliance to standards.
• Participate in quality improvement and population health initiatives.
