Job summary

Oak Street Health seeks a Nurse Practitioner for outpatient, Medicare-focused primary care. The NP works with a physician/PA team, nurses, scribes, and care managers to provide longitudinal care and build patient relationships.

Qualifications

• Master's or Doctoral nursing degree or NP certificate.
• Active NP license and DEA license as required.
• US work authorization required.
• Board certification in one of the listed NP specialties.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer wellness visits and health risk assessments.
• Care coordination with providers, labs, and facilities.
• Population health leadership with the care team.
• Assist with phone triage and outreach.
• Educate patients on conditions, plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
• Reports to Center Medical Director and collaborates with leadership.
