Job summary

Millennium Physician Group in Florida is seeking a Nurse Practitioner or Physician Assistant to deliver high-quality, patient-centered primary care within our value-based care model. The NP/PA will work autonomously at the top of their license and collaborate with a multidisciplinary team to improve health outcomes.

The role emphasizes preventive care, chronic disease management, and population health initiatives. Requires masters/NP or DNP, board certification, and Florida licensure.

Qualifications

• Master's or Doctor of Nursing Practice from an accredited program.
• Current, unrestricted NP license and APRN licensure in the applicable state.
• National board certification as FNP/AGPCNP/AGACNP or equivalent.
• DEA registration and BLS certification.
• ACLS preferred.
• 2+ years of APRN experience in primary care or related field.

Responsibilities

• Evaluate, diagnose, and manage patients with acute and chronic conditions.
• Develop individualized treatment plans including medications and preventive care.
• Order and interpret labs and imaging to support clinical decisions.
• Conduct wellness visits and preventive screenings.
• Collaborate with physicians and care team to coordinate patient care.
• Educate patients and caregivers to support self-management and wellness.
