Job summary

Millennium Physician Group in Florida is seeking a Primary Care Advanced Practice Provider (APRN or PA) to join a physician-led, patient-centered team focused on value-based care.

You will partner with physicians and care teams to deliver comprehensive preventive and chronic care, manage complex cases, and help improve outcomes while reducing unnecessary ER visits. This role offers autonomy and supportive infrastructure to impact patients' health.

Qualifications

• Active Florida APRN or PA license in good standing.
• National Board Certification as APRN or PA.
• Current DEA registration or eligibility to obtain.
• Experience in Primary Care, Family/Internal Medicine, geriatrics or related field.
• Strong clinical assessment and chronic disease management skills.
• Experience with AthenaOne EMR is a plus.

Responsibilities

• Provide comprehensive primary care services for assigned patients in collaboration with physicians and interdisciplinary care team members.
• Perform patient assessments, physical examinations, and comprehensive care planning.
• Evaluate, diagnose, and treat acute and chronic medical conditions using evidence-based clinical guidelines.
• Develop risk management plans and provide chronic care management and palliative care services within scope of practice.
• Interpret laboratory studies, imaging results, and diagnostic findings to guide treatment decisions.
• Prescribe medications and order home health services, therapies, and DME as appropriate.
• Identify and address quality measures, care gaps, and HCC documentation opportunities to support value-based care initiatives.
• Provide interventions that help prevent unnecessary emergency department visits and hospital admissions.
• Educate patients and caregivers on disease management, medication adherence, symptom management, preventive care, and advanced care planning.
• Coordinate care across primary care providers, specialists, caregivers, home health agencies, skilled nursing facilities, and community resources.
• Identify patients who would benefit from skilled nursing, therapy services, behavioral health support, social services, or other community-based resources.
• Ensure accurate, timely, and compliant documentation and billing within the electronic medical record.
• Collaborate with assigned Medical Assistants and practice leadership to support workflow efficiency, staff development, and quality improvement initiatives.
• Consult with supervising physicians regarding complex diagnoses, treatment plans, and patient management decisions as needed.
