Job summary

Millennium Physician Group is seeking a Primary Care Advanced Practice Provider (APRN or PA) to deliver total patient care in collaboration with the physician and care team in Florida. The APP will assess, plan, and implement interventions across acute, chronic, and palliative needs, with strong emphasis on continuity of care and patient education.

Florida license in good standing is required, along with a master’s degree in nursing for APRNs.

Qualifications

• Active Florida APRN or PA license in good standing.
• Master’s degree in nursing (for APRN) or equivalent PA qualification.
• Experience in primary care/acute or chronic care management preferred.

Responsibilities

• Provide total care of assigned patients in collaboration with physician and care team.
• Perform patient assessments, care planning, and provide acute, chronic, and palliative care.
• Diagnose, interpret tests, prescribe medications, and order home health or therapies.
• Coordinate with Palliative/Hospice and document/bill promptly.
• Communicate with Primary Care Provider to ensure continuity of care.
• Instruct patients on treatment plans and promote evidence-based medicine.
