Job summary

Millennium Physician Group in Florida is seeking a Primary Care Advanced Practice Provider (APRN or PA) to deliver comprehensive, patient-centered care within a collaborative, value-based care model. You'll work with physicians and care teams to manage acute, chronic, and palliative needs, focusing on preventive care, quality outcomes, and efficient clinical workflow.

Ideal candidates bring Florida APRN/PA license with board certification and DEA, plus experience in primary care and EHR systems

Qualifications

• Active Florida APRN or PA license in good standing
• National Board Certification as APRN or PA
• Current DEA registration or eligibility to obtain
• Experience in Primary Care/Family Medicine/Geriatrics preferred
• Strong clinical assessment and chronic disease management skills
• Knowledge of value-based care, population health, quality measures, and HCC documentation
• Experience with AthenaOne EMR is a plus

Responsibilities

• Provide comprehensive primary care services for assigned patients in collaboration with physicians and care team members
• Perform patient assessments, physical examinations, and comprehensive care planning
• Evaluate, diagnose, and treat acute and chronic medical conditions using evidence-based guidelines
• Develop risk management plans and provide chronic care and palliative care within scope
• Interpret laboratory results, imaging, and diagnostics to guide treatment
• Prescribe medications and order home health services, therapies, and DME as appropriate
• Identify and address care gaps and HCC documentation opportunities to support value-based care
• Educate patients and caregivers on disease management, adherence, preventive care, and advanced care planning
• Coordinate care across primary care, specialists, home health, SNFs, and community resources
• Identify patients who would benefit from skilled nursing, therapy, behavioral health, or social services
• Ensure accurate documentation and billing in EHR
• Collaborate with Medical Assistants and leadership to improve workflow and quality
• Consult with supervising physicians on complex cases and treatment plans
