Millennium Physician Group is seeking a Nurse Practitioner or Physician Assistant to deliver comprehensive primary care within a value-based care model. You will diagnose, treat, preventive care, and manage chronic conditions with autonomy and collaboration with physicians and care teams.

The ideal candidate embraces patient advocacy, evidence-based practice, and continuous quality improvement, contributing to population health and care coordination across the continuum.

Formación

• Master's or Doctor of Nursing Practice degree from an accredited NP program.
• Current, unrestricted NP license in the applicable state.
• National board certification as FNP/AGPCNP/AGACNP or equivalent.

Responsabilidades

• Evaluate, diagnose, and manage patients presenting with acute illnesses, chronic diseases, and preventive healthcare needs.
• Develop individualized treatment plans including medication management, preventive services, immunizations, and lifestyle counseling.
• Order, perform, and interpret laboratory studies, imaging, ECGs, and point-of-care testing.
• Conduct annual wellness visits and chronic care management.
• Coordinate referrals to specialty care and ancillary services when indicated.
• Educate patients and caregivers on diagnoses, medications, and self-management.

Educación

Master's or Doctor of Nursing Practice degree

Current, unrestricted Nurse Practitioner license

National board certification (FNP/AGPCNP/AGACNP)
