Millennium Physician Group in Florida seeks a Nurse Practitioner or Physician Assistant to deliver high-quality primary care within a value-based care model. You will evaluate and manage patients, develop care plans, and coordinate with physicians and care teams to improve outcomes.

The role offers clinical autonomy with physician collaboration as needed, emphasis on preventive care, and population health initiatives. Experience in primary care and board certification are preferred.

Formación

• Master's or Doctor of Nursing Practice degree from an accredited program.
• Current, unrestricted NP license and ARNP licensure in the applicable state.

Responsabilidades

• Evaluate, diagnose, and manage patients with acute illnesses, chronic diseases, and preventive care needs.
• Develop individualized treatment plans including medication management and preventive services.
• Coordinate referrals and collaborate with physicians, nurses, and care teams to ensure comprehensive care.
