Job summary

One Medical in Hoboken, NJ is seeking a qualified Family Nurse Practitioner or Physician Assistant to join our outpatient primary care team. You will deliver high-quality, human-centered care across in-office visits, testing centers, and tele-health sessions, while collaborating with a broad care team to coordinate patient needs.

The role requires NJ license at start, and experience in outpatient primary care.

Qualifications

• Completed an accredited FNP or PA program with national certification.
• NJ state license required by start date.
• Past 5 years: 2+ years outpatient primary care with all ages, or 1 year plus 1 year fellowship or 1+ year urgent care.

Responsibilities

• Manage a patient panel with diverse needs across acute, chronic, and well visits.
• Treat patients in-office, testing centers, and conduct tele-health visits.
• Participate in weekly Clinical Rounds and other learning modalities.
• Collaborate with in-office and virtual teams; perform in-office procedures as eligible.
