Job summary

Upperline Health Plus is seeking a Primary Care Medical Assistant to deliver high-quality, patient-centered care in a value-based practice. You will support the Primary Care Provider, coordinate care, and help improve patient outcomes.

In this on-site role in Hixson, TN, you will room patients, obtain histories, take vital signs, document in the EHR, and collaborate with a multidisciplinary team to ensure smooth clinic operations and exceptional patient experience.

Qualifications

• Certified MA or 2+ years in a medical primary care setting.
• Primary care/outpatient clinic experience preferred.
• MA or phlebotomy certification strongly preferred.
• Knowledge of medical terminology, disease processes, pharmacology.
• Ability to interpret provider documentation and orders.
• EMR experience, AthenaHealth.
• Proficiency in Word and Excel.

Responsibilities

• Room patients, obtain vital signs, medical history, and chief complaints.
• Assist NP with health assessments and visits for Upperline Plus patients.
• Coordinate care and outreach to improve outcomes.
• Document clinical information in the EHR accurately.
• Perform medication reconciliation and review prescriptions.
• Handle telephonic outreach and care coordination tasks.
• Manage messages, refills, and prior authorizations.
• Support workflow and patient flow in the clinic.
• Collaborate with providers, nurses, care managers, and social services.
• Identify barriers to care and escalate as needed.
