UnitedHealth Group is seeking a Pride+ Care Adult Facilitator to support LGBTQIA+ patients within primary care teams, coordinating care and serving as a liaison among clinicians, social workers, and family members.

You will maintain longitudinal patient relationships, assess needs, and refer to community resources aligned with identity and values. The role emphasizes data entry, reporting, and participation in care improvement activities.

Formación

• High School Diploma/GED required.
• 1+ year experience in a healthcare setting with patients.
• Experience with LGBTQIA+ patient populations is a plus (preferred).
• Bachelor's degree in social work or population health management preferred.

Responsabilidades

• Serve as a primary contact for LGBTQIA+ patients and provide tailored support.
• Advocate for patients and help overcome barriers to care.
• Coordinate with care teams, specialists, and insurers as needed.
• Track appointments, labs, studies, referrals, and medication adherence.
• Lead monthly panel roster reviews and participate in care navigation meetings.
